Short summary
In preparation for the upcoming release – and subsequent distribution – of the Polkadex Crowdloan PDEX rewards, the Polkadex core contributors have compiled on-chain (publicly available) contribution data obtained via explorers and off-chain contribution lists provided by DApp contribution partners, Equilibrium, Bifrost, and Parallel Finance. 
Thanks to this, each crowdloan contributor will be able to claim their rewards in the same way, no matter the method they used when originally contributing to the crowdloan, and without having to go through a third party. Essentially, we’ve removed the need for any manual distribution of crowdloan rewards through centralised agents. 
For the purpose of transparency and to further corroborate the list of eligible addresses, we wanted to share the total on-chain contribution list along with the address lists we received from our contribution partners:

Polkadex Polkassembly thread (https://polkadex.polkassembly.io/post/2363) with all the contributor addresses and the amount of DOT each address contributed. 

Additionally, Polkadex has integrated the reward claiming UI (https://polkadex.trade/rewards) to their testnet for contributors to test their contributions before the rewards claiming page goes live. (Please note that the amount of claimable rewards will be significantly higher when you follow this process on mainnet as this is pro rated per block and the testnet doesn’t have the same number of blocks as the mainnet.)

Users who contributed through SALP can test their Contribution numbers and the rewards they're eligible for. You can respond on the Polkassembly thread if they see any discrepancies.

Note: You need more than one test PDEX in their account to cover the existential balance and transaction fees. You can use their token faucet (https://tokenmanager-beta.polkadex.trade/faucet) to get some test PDEX.
